The High-Risk Neonatal Follow-Up Program/Clinic (HRC) is a long standing, multidisciplinary primary care and neuro-developmental clinic for infants discharged from our NICU, network hospitals, and other hospitals with complex medical and social needs. The High-Risk Infant Follow-up Program/Clinic is currently located on the Stroger Hospital campus.
We strive to provide a system of services to families with young children, birth to five, who have developmental disabilities or who are at risk for developmental delays. Patients are generally followed up to age five, at which time some are referred to our Comp Care Clinic and those who meet the criteria are referred to the Medical Home.
The HRC clinic holds two half-day sessions each week: Tuesday PMs and Friday AMs, staffed by neonatologists and fellows. Infants are assessed at regular intervals and the neonatologists provide primary care and coordinate specialty care such as neurology, ophthalmology, genetics, cardiology, surgery, urology, radiology, nutrition, social work, and early intervention services.
