Inpatient Services
Emergency Services and Inpatient Care are no longer provided at Oak Forest Health Center.
Outpatient Services
For appointments, call (708) 633-4293. Physician referrals are required for services. OFHC also has an on-site Spanish interpreter that we can page for your convenience.
E Building
Most clinics are located in the E Building, the outpatient services ‘hub’ on the Oak Forest Health Center campus.
Services include:
- Primary care
- Cardiology
- Nephrology
- Gastroenterology
- Infectious Disease
- Pain Management
E Building Clinic hours are 8:00 AM until 4:30 PM, Monday through Friday. Tuesday clinics are open until 6:30PM.
H Building
H Building Clinics are Podiatry, Optometry, Radiology and Rehabilitation Services, including:
- Physical Therapy, Occupational Therapy, Speech/Audiology. Physical Therapy clinic specialized sessions include: Wheelchair, Prosthetics and Musculoskeletal Clinics.
H Building Clinic hours are 7:30 AM until 3:30 PM, Monday through Friday.
F Building
The F Building contains Laboratory and Pharmacy Services.
B Building
The B Building has Sleep Lab services, for treatment of sleep-related issues and disorders.
