Under the supervision of the Chief Psychologist, plans, coordinates and implements the case management, jail diversion and linkage activities for mentally ill detainees at Cook County Department of Correction (CCDOC). Maintains continuity of care and helps ensure detainees receive care in the last restrictive environment.
A Master’s Degree in Social Work from an accredited School of Social Work is required. (Must provide original/college university transcript at time of interview).
Four (4) years of full-time experience in case management in a mental health environment is required.
Must be a licensed clinical social worker (LCSW) in the State of Illinois. (Must provide license at time of interview).
Must possess two (2) years of full-time work experience providing social work services at the LCSW level.
MUST MEET ALL REQUIRED QUALIFICATIONS AT TIME OF APPLICATION FILING.
Knowledge, Skills and Abilities
Thorough knowledge of: social work practices, procedures and techniques; medical terminology and procedures; and mental health treatment services. Ability to: plan, direct and evaluate the professional and administrative aspects of social work services; communicate effectively, verbally and in writing with a wide variety of professional and lay individuals and mentally ill patients; provide direction and supervision to subordinate employees and trainees; and formulate and develop social services, jail diversion and linkage policies, procedures and standards. Skill in: utilizing the DSM and other diagnostic instruments, planning, directing and evaluating the professional and administrative aspects of case management services; and conducting bio-psychosocial interviews.
